If you own an animal that you are hoping to surrender to BCHS, please complete the following form. Staff will then review your form and email or text you regarding availability and next steps. There is a $100 fee per surrendered pet.

Please be aware there is no guarantee that we can pull your pet. This form helps us assess what resources and support we can give you as well as begin the process to surrender. We prioritize Bartholomew County residents and wait time varies based on our open kennel space.
